Comparative study of the management of inter-trochanteric fractures in the elderly: Trochantric femoral nail vs dynamic hip screw

Introduction: Trochanteric fractures are among the most common injuries necessitating hospital admission. Regardless type of fracture, trochanteric can lead to substantial morbidity and mortality, especially in elderly patients. Surgery has been mainstay treatment for these allow early mobilization patient. Among surgical treatment, dynamic hip screw (DHS) as extramedullary power transmission system fixation nail (TFN) means intramedullary stabilization established standard femoral fractures, particulary AIM: The present study was designed compare results TFN DHS intertrochanteric fracture femur. Material methods: After fulfilling mentioned criteria, patients were screened medical fitness then further randomized group I II. Group I-Patients treated with (n=30). II-Patients clinical assessment done according “Salvati Wilson Function Score (SWS Score)”. Blood loss measurement complications also studied compared. Results: Thirty included each group. Salvati (SWS) used evaluate at follow up. Mean ± SD SWS 16 weeks & 24 18.93±3.34 28.80±5.33 respectively whereas II, value 15.13±2.18 21.67±5.14 respectively. P-value significant (< .01) Complications observed ups frequent varus displacement. It that out 30 subjects 2, maximum i.e. 8 (26.67%) had deformity 1, only 1 (3.33%) subject Other include Non Union Implant failure. Conclusion: We conclude advantage is a smaller exposure required than sliding screw, it may therefore be associated lesser blood loss, shorter operating time less morbidity.
